Change of Heart is a One Nottingham commissioned project focusing on reducing inequalities in Cardiovascular Disease (CVD) within the Inner City Super Output Areas, which have disproportionately high levels of CVD. This project targets the whole community with some focus on the Diverse BME Communities. The project is funded by One Nottingham and will run for 18 months.
